Waiver For Kids and Teens Classes

I, the undersigned parent or legal guardian of [_________________________], hereby agree to the following terms and conditions for their participation in the kids pottery class:

1. I understand that pottery involves the use of clay and other materials that may cause allergic reactions in some children. If my child has any known allergies, I have informed the instructor beforehand and will provide any necessary medication or precautions as needed.

2. I understand that children with special needs may require additional assistance or accommodations during the class. If my child has any special needs, I have discussed them with the instructor and will provide any necessary support to ensure their safety and enjoyment during the class.

3. I agree to pick up my child promptly at the end of each class session. If I am unable to do so, I will notify the instructor in advance and provide emergency contact information for someone who can pick up my child in my absence.

4. In case of emergency, I authorize the instructor or other staff members to seek medical treatment for my child as necessary. I will provide updated contact information and notify the instructor of any changes to my child's health or emergency contacts.
